The Evidence-to-Action Hub builds on the foundation of the Better Care Playbook, an online resource library that launched in 2016 by the Institute for Healthcare Improvement to advance the adoption of evidence-based care for people with complex needs. In 2020, the Center for Health Care Strategies (CHCS) assumed leadership of the Playbook with support from seven leading health care foundations: Arnold Ventures, The Commonwealth Fund, The John A. Hartford Foundation, the Milbank Memorial Fund, Peterson Center on Healthcare, the Robert Wood Johnson Foundation, and The SCAN Foundation.

Now evolved into the Evidence-to-Action Hub, the resource continues to support evidence-based care for people with complex needs, while refining its focus to highlight proven strategies for improving care across populations served by Medicaid. 

CHCS gratefully acknowledges the generous support of the founding funders from 2016 through 2025. Their vision brought this resource to life and continues to guide its future.
