Resources for People with Behavioral Health and Social Needs

People with complex medical needs often have behavioral and social needs. This combination of needs dramatically affects the interventions they should receive and the care team composition. For instance, people living with a serious mental health condition (e.g., schizophrenia) are more likely to experience poor health outcomes due to uncoordinated care and limited access to appropriate services. Social needs — including unstable housing, low socioeconomic status, and lack of supportive relationships — also exacerbate poor health and quality-of-life outcomes.
