Why Redesign Care for People with Complex Needs?

When adults with complex needs require medical services, they often encounter a health care system that is poorly coordinated, inefficient, and expensive. They often face many challenges, such as chronic conditions, behavioral health issues, trauma history, food insecurity, and homelessness. Health plans, accountable care organizations, and providers cannot achieve their quality and cost goals without improving care for people with complex needs. The broad implementation of care models for this population is critical to improving outcomes and lowering costs.
