Context

Caregivers often do not receive the necessary information and training to provide care at home for their family members with complex needs. This series of resources was designed to help nurses understand how they can support caregivers as well as provide videos and fact sheets that nurses can share with family caregivers to educate them about performing certain tasks.

About this Tool

Topics addressed in this series include: complex medication management, fall prevention and safe mobility, management of urinary incontinence, use of mechanical lifts, prevention of home dialysis complications, ostomy care, dietary and feeding modifications, and wound and skin care. There are also articles to educate nurses and family caregivers on the 4Ms framework of an Age-Friendly Health System.
