Evaluation of a diabetes-focused medically tailored meals (MTM) pilot demonstrates improved health outcomes and provides examples of key implementation tools.
Context
MTMs have been shown to improve outcomes for individuals with chronic health conditions, such as diabetes. This evaluation analyzed outcomes related to a 12-week MTM pilot implemented in Solano County, California, which provided 51 older adults who have diabetes with ten prepared meals weekly, along with grocery boxes and diabetes education.
Findings
Among the 33 participants who completed pre- and post-intervention assessments, the majority showed improved diabetes management: 91% demonstrated reduced HbA1C levels and 70% showed improved blood glucose measurements. Additionally, 76% reported improved eating habits.
