Context

MTMs are meal preparation programs designed for people with serious or chronic conditions. MTMs improve health outcomes, lower health care costs, and increase patient satisfaction. MTM program models, however, vary depending on the range of services offered, target populations, and structure. This resource summarizes findings from a convening hosted by the National Resource Center on Nutrition and Aging on the state of MTM research and profiles three different MTM program models.
